Procedures:
Follow these steps to make registration fast and easy:

1) DOWNLOAD all the pertinent forms from the Forms page.

2) READ the Registration Agreement, Participation & Release Agreement, Medical Information Form and Parents' Guidebook and Reference Manual. Also read the Acknowledgement form and the Credit Card Authorization form.

3) CALL SwimJim, Inc. (212-749-7335) and speak with a Client Service Associate who will help find the right class at the most convenient time and location for you.

4) SIGN the Registration Agreement, Participation & Release Agreement, and acknowledgment then complete the Medical Information Form for your child. You must also pay a non-refundable $35 registration fee per child to secure your child's place. This fee is paid only once per child per calendar year. If you wish to pay this fee by credit card, complete the Credit Card form too.

5) FAX these completed forms back to us and keep your confirmation (Please make sure you mail them or bring them to us/class). We will then call you to confirm your child's class. The SwimJim, Inc. Parents' Guidebook and Reference Manual is your resource tool; there is no need to fax it back to us.
